New Patient Registration

Anyone wishing to register at the surgery should telephone or present in person at the surgery during surgery hours and ask to speak to a member of the reception team who will give you the appropriate paperwork to complete. You will need to bring 1 x piece of photographic ID [eg passport, driving licence] to complete your registration. You will also be asked to complete a patient questionnaire, which will also ask if you have a preference of practitioner.

It will help the process if you have the details of your last GP and your NHS number, but this is not essential.

You will be offered the opportunity to book an appointment for a new patient health check, either with you preferred practitioner or one of the GPs if you have no preference. Reception staff will be happy to assist with any queries you might have.
